Why did the health care bill fail to address cost and cost containment of premiums?

Yes we can add our children to an older age, and add a spouse who has special conditions but the price for these policies is so expensive that a working person cannot afford them. Cobra payments are out of this world and not affordable if you don't have a job...What is the Democratic anser to this.
